The official state program is Delaware Medicaid & Medical Assistance. Contact the state or your health plan to confirm eligibility, covered services, required notice, and the correct ride contact for your plan or location.

Delaware DMMA contracts with Modivcare for statewide non-emergency transportation to covered services. Eligible Delaware Medicaid and Delaware Healthy Children Program members can call 1-866-412-3778 and should arrange a ride at least three days before the appointment. Modivcare verifies eligibility and the covered service before arranging the trip.
