Indiana covered rides
The official state program is Indiana Family and Social Services Administration. Contact the state or your health plan to confirm eligibility, covered services, required notice, and the correct ride contact for your plan or location.
Indiana Traditional Medicaid, or fee-for-service, members can call the state-published NEMT reservation line at 1-855-325-7586 to arrange transportation to covered medical services. The state asks members to call at least two business days ahead. Members in HIP, Hoosier Care Connect, Hoosier Healthwise, or Indiana PathWays for Aging should use their managed-care organization’s transportation contractor instead.
Route details: Traditional Medicaid fee-for-service members use the state-published Verida route. HIP, Hoosier Care Connect, Hoosier Healthwise, and Indiana PathWays for Aging members use their MCO’s designated contractor. Verida or the MCO determines individual trip handling.
