The official state program is Kentucky Cabinet for Health and Family Services. Contact the state or your health plan to confirm eligibility, covered services, required notice, and the correct ride contact for your plan or location.

Kentucky Medicaid directs members to the Human Service Transportation Delivery (HSTD) Brokerage Listing to find the transportation broker for their region. The broker can explain the request process and appointment details.

Route details: Find the member’s region in the official HSTD Brokerage Listing, then contact the broker listed for that region. For specialty care or care outside the member’s medical service area, the state says a primary-care referral is required.
