The official state program is MassHealth. Contact the state or your health plan to confirm eligibility, covered services, required notice, and the correct ride contact for your plan or location.

For MassHealth Standard, CommonHealth, or CarePlus, ask your medical provider to submit an online PT-1 transportation request; if it is approved, schedule with the broker assigned for your city or town. One Care, Senior Care Options, and PACE members should contact their plan directly. MassHealth Customer Service at 1-800-841-2900 can answer transportation eligibility or PT-1 status questions.
