Minnesota covered rides

The official state program is Minnesota Department of Human Services. Contact the state or your health plan to confirm eligibility, covered services, required notice, and the correct ride contact for your plan or location.

Minnesota members enrolled in a health plan should use the official DHS health-plan list to call that plan's nonemergency medical transportation contact. Members with fee-for-service coverage, also called straight MA, should use the DHS fee-for-service transportation materials and their county or Tribal office for the applicable process.

Route details: Use a plan-specific NEMT number only for the member’s enrolled plan. Members without a health plan use the official fee-for-service materials and county or Tribal route; the county or Tribal office determines individual NEMT handling.
