New Hampshire covered rides
The official state program is New Hampshire Department of Health and Human Services. Contact the state or your health plan to confirm eligibility, covered services, required notice, and the correct ride contact for your plan or location.
New Hampshire Medicaid members enrolled in a health plan should use the route for that plan: WellSense 1-844-909-7433; NH Healthy Families/MTM 1-888-597-1192; or AmeriHealth Caritas New Hampshire/CTS 1-833-301-2264. Fee-for-service members should contact New Hampshire Medicaid member help using the state link below to identify their current transportation route. Ask your plan or the state about advance notice and any trip limits.
Route details: Check the member’s plan card. Use the matching WellSense, NH Healthy Families, or AmeriHealth Caritas New Hampshire contact when enrolled in that plan. A fee-for-service member uses New Hampshire Medicaid member help to identify the active route.
