The official state program is Ohio Department of Medicaid. Contact the state or your health plan to confirm eligibility, covered services, required notice, and the correct ride contact for your plan or location.

Ohio’s current transportation assistance guide says managed care and MyCare Ohio members should contact their plan or the Ohio Medicaid Hotline. Members outside those plans can contact the Medicaid Transportation Coordinator at their county CDJFS; wheelchair-van provider search is a separate option.
