Utah covered rides
The official state program is Utah Department of Health and Human Services (Utah Medicaid). Contact the state or your health plan to confirm eligibility, covered services, required notice, and the correct ride contact for your plan or location.
Utah’s NEMT page is for Traditional Medicaid. Members outside an area served by bus or paratransit, or who need door-to-door service, may be eligible for ModivCare at 1-855-563-4403. Utah asks members to schedule at least three business days before an appointment and lists separate local transit and paratransit options where available.
