The official state program is Vermont Health Connect / Department of Vermont Health Access. Contact the state or your health plan to confirm eligibility, covered services, required notice, and the correct ride contact for your plan or location.

Vermont Medicaid members can call the Vermont Public Transportation Association at 1-833-387-7200 to ask about a ride with their area provider. This route is for pre-scheduled Medicaid-billable appointments, including prescription pickup, when transportation is not otherwise available and the member does not live on a public bus line.

Route details: Use the VPTA contact for a pre-scheduled appointment with a participating Medicaid provider or for prescription pickup when the member lacks available transportation and is outside a public-bus-line route.
