Wisconsin covered rides
The official state program is Wisconsin Department of Health Services. Contact the state or your health plan to confirm eligibility, covered services, required notice, and the correct ride contact for your plan or location.
Wisconsin Medicaid directs members to its NEMT manager at 1-866-907-1493 (711 TTY) to book, change, cancel, or check a trip. The official MTM Link portal also supports online trip management; Wisconsin says first-time users and new appointment locations must be scheduled by phone.
Route details: DHS lists BadgerCare Plus, Wisconsin Medicaid, Family Planning Only Services, and TB-Related Only Services among programs whose members may use statewide NEMT when they have no other transportation. The NEMT manager determines individual trip handling.
